What Is Required for An Immigration Physical Exam?
Our physicians at North Dallas Primary Care Doctors are registered to provide INS physical examinations, immunizations, and other healthcare-related services as governed by the United States Citizenship and Immigration Services (USCIS). We welcome you to make an appointment or call for a walk-in appointment to have your immigration exam completed. Before arriving at our medical facility for your exam, please make sure you bring a valid picture government ID (driver's license or passport, which cannot be expired), your I-693 paperwork (download at uscis.gov), and your official shot record (without proof of immunizations, you must receive all required vaccines).
We recommend that you print out and bring the most recent version of Form I-693, Report of Medical Examination and Vaccination Record, to your medical exam appointment at North Dallas Primary Care Doctors. Please carefully fill out the paperwork accordingly, completing Part 1 of the I-693 form, but do not sign your name until our medical team instructs you to do so. This form will be used to document and detail the results of your immigration medical exam.
